The Cortisol Stimulation by ACTH Test evaluates adrenal gland function by measuring cortisol response after synthetic ACTH administration. This dynamic stimulation test is the gold standard for diagnosing primary, secondary, and tertiary adrenal insufficiency, and may also help identify congenital adrenal hyperplasia and assess recovery from prolonged corticosteroid use.
Overnight fasting (at least 8 hours) is mandatory. Synthetic ACTH (Synacthen) is not supplied by our laboratory. During the test, a fasting baseline blood sample is drawn, then 250 ยตg of synthetic ACTH is administered intravenously. Additional blood draws occur at exactly 30 and 60 minutes post-injection. All tubes must be clearly labelled with collection times and submitted on one test request form. Samples must be refrigerated or frozen during transport.
