Sample Condition: 3 mL whole blood in 1 Blue Top (Sodium Citrate) tube. Mix thoroughly by inversion. Transport to Lab within 4 hours. If this is not possible, make PPP within 1 hour of collection as follows: Centrifuge sample at 3600 rpm for 15 min. & transfer supernatant to a clean plastic tube. Centrifuge this supernatant again at 3600 rpm for 15 min. & finally transfer the supernatant (PPP) to 1 labelled clean plastic screw capped vial. FREEZE IMMEDIATELY. Ship frozen. DO NOT THAW. Overnight fasting is preferred. Duly filled Coagulation Requisition Form (Form 15) is mandatory.

- Activated Partial Thromboplastin Time (aPTT): This test measures the time it takes for blood to clot. In the presence of lupus anticoagulant, the aPTT is prolonged.
- Dilute Russell Viper Venom Time (dRVVT): This test is similar to the aPTT but uses a specific reagent derived from a snake venom. It is more sensitive in detecting lupus anticoagulant.
- Platelet Neutralization Procedure (PNP): This test is used to confirm the presence of lupus anticoagulant by neutralizing the anticoagulant activity of patient plasma with normal platelets.
- Hexagonal Phase Phospholipid Neutralization (HexPhos): This test is used to detect lupus anticoagulant by neutralizing the anticoagulant activity of patient plasma with hexagonal phase phospholipids.
- Confirmation Testing: If the initial screening tests are positive, additional confirmatory tests may be performed, such as mixing studies or specific factor assays.
The LAC PANEL test is ordered when a patient presents with symptoms of abnormal blood clotting, such as deep vein thrombosis or recurrent miscarriages. It is important to note that a positive LAC PANEL test does not necessarily mean the patient has lupus, as lupus anticoagulant can be found in other autoimmune diseases and even in healthy individuals.
